Who may request Sacrament Certificates | Quien puede pedir certificados sacramentales

Sacramental records and certificates may only be requested by parents or legal guardians of a minor. For Adults, they may request their sacramental records or certificates.
We will NOT share information or give records to other individuals whose records do not belong to them. For special cases, we will consult the Archdiocese Records department.

For out-of-area requests, they must first communicate with the office, fill out the information below, and provide a scan of their current ID/Driver's license to verify identity.
Once verified, we may mail certificates via regular USPS.

We may send certificates from Church to Church via fax, email, and USPS upon request of the Coordinator or Priest.

Please keep in mind that requests are made with time, we thank you for your understanding and patience.
